This is one of the most intensive levels of care as staff members are available 24/7, but it is better than inpatient care because the environment is peaceful at home as opposed to the potentially busy chaos of the hospital wing.

Residential care focuses on a holistic approach to healing and allows the time needed to do so. Where inpatient treatment in a hospital mainly focuses on the physical stabilization of a person, or talk therapy mainly focuses on improving mental health over time, residential treatment provides both at the same time. Patients will enter an environment intimate enough to receive a range of treatments – individual and group therapy – in a real-life environment. Our homes are designed in a way that supports a safe, supervised and home-like environment to help alleviate the symptoms/crisis that lead to the need for this level of intensive care. We focus on helping our patients gain an understanding of what led to RTC treatment, return to a pre-crisis state, and develop skills and strategies to prevent the need for future readmissions. Treatment of a maximum of 12 patients at a time per home is tailored to the needs of each person and family and/or support system. The low patient-to-staff ratio (6:1 therapist to case manager) allows for more individual attention and engagement in this unique location, which features a swimming pool, nature trails, and koi ponds. Typically, our patients are treated for 4-6 weeks.

In Annapolis, we provide trusted and proven treatment modalities such as trauma-focused cognitive behavioral therapy (CBT), dialectical behavior therapy (DBT), acceptance & commitment therapy (ACT), desensitization & eye movement response therapy (EMDR), and more. Patients also participate in Conscious Recovery, art therapy, equine therapy, wellness components and cellular meditation.

When patients first arrive, they are greeted at the door by their case manager and assigned a licensed therapist to support their goals. They will receive a full clinical, nursing and nutritional assessment to ensure suitability. Patients will receive support with a medication management program overseen by our board-certified psychiatrist, 24-hour nursing care, and a nutrition program designed by a dietitian. Additionally, patients will receive support from a dedicated UR team to help navigate their insurance. This also allows the therapist to focus on their clinical care and build a therapeutic alliance.

As patients progress through their treatment, a staging system based on stages of change is used, and we track their progress through evidence-based tools such as PHQ-9, RAS-41, and DASS-21.

O In this phase of treatment, people adapt to residential treatment and program structure. This is the stage of change where they decide what changes in life and what it looks like for them. All patients were in the pre-phase for at least 72 hours.

O At this stage of the journey, patients have accepted their need to participate in treatment, recognize their own importance and determine the areas of change needed in their lives. In this phase of treatment, they begin to take steps towards their goals.

O In this phase of treatment, patients are actively engaged in therapy and include what is discussed in therapy sessions to move to recovery. They have become more aware of the patterns or causes of their struggles and demonstrate the ability to achieve goals. Eventually, patients are better able to cope with daily stress and show independence in managing their emotions.

The patient is no longer involved in behaviors reminiscent of past actions and can freely use alternative behaviors or skills in the moment. Patients can generalize skills in family sessions, handovers, outings, etc. and require little support from staff in the environment.

Nicole Swann is a licensed clinical social worker in Maryland and Texas. She is a graduate of Texas State University, where she earned a master's degree in social work with a focus on administrative leadership. She has worked in various environments including child welfare, homelessness, crisis situations and hospital programs (PHP & IOP). The experience is aimed at adolescents and adults in reducing immediate crises, developing coping skills, adapting to life challenges, increasing distress tolerance and improving emotion regulation. She takes an eclectic approach to therapy, using a combination of short-term, goal-oriented, strength-based and solution-focused therapy strategies to adapt her therapy style to support the needs of the individuals she works with. He comes from a trauma-informed perspective and most often uses dialectical behavioral therapy (DBT), cognitive behavioral therapy (CBT) and motivational interviewing (MI). Outside of work, Nicole is very competitive, loves sports and can be found spending time with her family.
