Your out-of-pocket costs may be higher with a PPO than with an HMO or EPO plan. You will have doctors and hospitals in the network (depending on the network) that have contracts with insurance companies to accept lower amounts. Or, you can look at a provider outside the network, but you will pay more from the network provider. You can view in-network and out-of-network providers if you wish.

EPO can be described as a combination of PPO and HMO. You don't have to choose a general practitioner, and you can look at any provider in the EPO network, but there are no immediate benefits. If you choose treatment outside your plan's network, you will pay the full cost, no coverage, except in the event of an accident. If you're looking for lower monthly payments, and aren't comfortable using local doctors and hospitals, but don't want referrals to specialists, you may want to consider an EPO plan.
